The SEEDS Program: A Residential continuum of care to help Crawford County adults with mental health diagnoses learn and develop skills to live independently, build confidence and belief, overcome obstacles, and build a support team. This program provides the missing steps between 24/7 residential homes and independent living.
SEEDS is a time-limited program using residential homes in the Meadville Community to help individuals learn the skills to live independently. It is not a housing service. Activities are very goal-directed towards moving through the program and out into individual homes or apartments.

A goal-oriented environment with encouragement and support, opportunities to grow, obstacles and challenges, all the while living life as a neighbor, tenant and friend.

12-Hour Support: Mental Health Supported Living–This home is the first step-down in services from 24/7 to 12-hour support. The residents at this level are demonstrating greater independence and continue to develop skills.
24/7 Support: This home is the usual entry point in the program where the person needs 24/7 support to learn the skills to become more independent.


6-Hour Support: Transitional Living–This home is the final step in the program before the resident graduates to independent living. The step-down is from 12-hour support to 6-hour support with the focus being primarily on seeking independent housing and supports in the community.
