Residential Rehabilitation Program
The Residential Rehabilitation Program provides group homes in the community in order to provide personal choice, empowerment and recovery for adults with serious mental health challenges. Each home allows for a maximum of 3 adult residents as well as block shift staff who rotate shifts through the week. This allows for a small staff to resident ratio, and therefore lots of opportunities to learn new skills, get out into the community and work towards greater independence.
The objectives of Residential Rehabilitation are:
1. to support individuals to remain in the community
2. to develop individualized recovery plans
3. to reduce risks associated with re-hospitalization
4. to assist individuals in working toward independent living if that is what they desire.
Referrals to the program are made through Service Coordinators/Case Managers from the County Human Services Department.
